Previously verified NSSAR membership and supplemental applications can be used as genealogical research tools and potentially as documentation for new applications. Copies of previously verified NSSAR membership applications (record copies) can be ordered using the various forms below.  Please select the appropriate form for your request.

The NSSAR has replaced the record copy form with 3 new forms.  Each of the new forms serves a particular type of request.

Requests can be broken down into the following:

• A request for information about a specific Patriot / Ancestor.  This type of request will provide you with the most recent application on file with the NSSAR for a given patriot.
• A request for a Specific members application.  This type of request will provide you with a copy of the specific members application.
• A request for the documentation for a particular application.  This type of request will provide you with a copy of the paperwork that was used to document an application.  The first 5 pages are included in the price, additional pages beyond 5 will incur additional charges.

Please be aware that we do not have backup documentation for those applications filed before 1978. The approximate Member National Number cutoff for the documentation files is numbers prior to #114000
